Pay Rate: £16.37 – £18.00 per hour
Hours: 37 hours per week
Duration: Temporary, 12 weeks
We are seeking a dedicated and compassionate Leaving Care Support Worker to join our team for a temporary 12-week assignment. This role focuses on supporting care-experienced young people as they transition from care to independence.
Key Requirements:
- Qualifications: A valid DBS check is required.
- References: Two professional references are mandatory.
- Experience: 6 months in the same or similar role within the UK
- Travel: Own vehicle and willingness to travel as part of the role.
Responsibilities:
Provision of Support Services:
- Deliver leaving care support services through the Pathway Assessment and Planning Process.
- Focus on areas including education, training, employment, accommodation, health, personal support, family and social relationships, practical skills, and financial assistance.
- Provide guidance, advice, and counselling to care-experienced young people to enhance their welfare and independence.
Case Management:
- Manage a caseload and develop, monitor, and review Pathway Plans in collaboration with service users.
- Respond to requests for support, gathering information to determine the level of assessment required.
Pathway Assessments:
- Complete Pathway Assessments in a timely manner, working closely with young people, carers, and social workers.
- Ensure Pathway Plans highlight individual strengths, development areas, and actionable steps for independence.
- Record young people’s and professionals’ views explicitly within assessments and plans.
